Timeline
- January 3 – Commitment packages due (you need to have one parent/guardian and one student commitment form submitted for each student attending the trip)
- January 3 – $500 Deposit Due* (see payments section)
- January 18 – Registration deadline on Kaleidoscope Adventures (see post for instructions)
- February 23 – Remaining $500 balance due
- February 23 – Additional forms due
- Field Trip Authorization Form
- Luggage and Lodging Search Permission Form
Forms can be scanned and signed and then emailed to springtrip@yorktownband.org or sent to Mr. Witkowski at school
- March 31 – Any additional medical authorization forms for prescriptions due
- Medication Authorization Form – one per medication student is taking to camp for self-administration
Doctor’s signature required for prescription medications but not over-the-counter medications

- April 25 – Depart for Boston!
- Report at YHS Band Room at 7:15 AM
- April 28 – Arrive in Arlington

Payments
Payment Schedule:
- Deposit Due ($500): January 3*
- Balance Due ($500) February 23
* In order to make this trip happen, the Yorktown Band Boosters will have financial obligations with our travel agent at the beginning of January. Due to the holiday timing, we are granting a “no questions asked” extension for the deposit until the end of January for families who need more time. If you are able to submit the deposit by January 3, however, we ask that you do so.
Cost should not be a barrier to participation, however students must have a zero balance on their financial statements with the band in order to be eligible for the trip. For assistance or alternate arrangements, please contact Mr. Witkowski.
